Reporting directly to the Director of Nursing, you will conduct initial telephone intakes, perform in-person assessments, update customized care plans, lead caregiver hands-on skills training, and conduct regular supervisory visits in client homes.

This is a dynamic, high-mobility role ideal for a licensed practical nurse who thrives on client advocacy, quality assurance, and community-based senior care.

What You'll Do

  • Supervisory Visits & Care Oversight: Conduct regular in-home supervisory visits and quality assurance assessments, with special focus on high-acuity (Tier A / 24/7 / live-in) clients, in alignment with State Department of Health regulations.

  • Caregiver Training & Mentorship: Lead hands-on practical skills training, competencies, and in-home re-trainings for caregivers. Partner with our Staffing Managers to ensure caregivers are confident and clinically prepared for each client's specific needs.

  • Incident Management & Quality Assurance: Rapidly address and resolve client or caregiver concerns, home incidents, or changes in condition, updating care plans and providing retraining as needed.

  • Care Coordination: Collaborate closely with the scheduling and staffing team to ensure the best caregiver-to-client match, verify service plan accuracy, and share care management duties with team peers.

  • Documentation & Regulatory Compliance: Maintain detailed, prompt client records and shift notes in our operating systems, ensuring full compliance with state home care licensure standards.

  • Community Engagement: Support local brand visibility by participating in community outreach, networking events, and collaborative initiatives with our Home Care Liaisons.
