Practice Medicine With Time to Care for the Whole Patient
Sutter SeniorCare PACE is seeking an experienced Internal Medicine, Family Medicine, or Geriatrics physician to join our interdisciplinary team in Sacramento. This is a different kind of primary care practice. You will care for medically complex older adults with longer appointments, approximately 6 to 10 visits per day, and a multidisciplinary team working alongside you. PACE, the Program of All-Inclusive Care for the Elderly, brings medical care, rehabilitation, behavioral health, social services, and other support together in one coordinated program. Our goal is to help older adults remain healthy, independent, and safely in their communities for as long as possible. What Your Practice Looks Like
- 6 to 10 patient visits per day
- Monday through Friday schedule, generally 8:00 a.m. to 5:00 p.m.
- 90-minute comprehensive assessments for new participants
- 60-minute semiannual reassessments
- 30-minute follow-up and urgent visits
- Daily interdisciplinary team collaboration and care planning
- Longitudinal relationships with a medically complex geriatric population
- Approximately 500 participants served across the Sacramento PACE program
- Resources spanning primary care, nursing, rehabilitation, behavioral health, social services, dental, optometry, podiatry, and audiology
A Truly Team-Based Model
At PACE, you are not managing complex older adults in isolation. Physicians work closely with nurse practitioners, nursing, medical assistants, social services, rehabilitation professionals, behavioral health clinicians, and other members of the interdisciplinary team. Daily team meetings are built into the practice. This creates dedicated time to coordinate care, anticipate changes in a participant's condition, and develop plans that incorporate medical, functional, behavioral, and social needs. For a physician who enjoys geriatrics, complex chronic disease management, goals-of-care conversations, or reducing unnecessary hospital utilization, PACE provides an opportunity to practice that work in a highly coordinated environment.Call
Call is shared among approximately six physicians and nurse practitioners.- Approximately one weekday call every 1.5 weeks
- Approximately every sixth weekend
- Weeknight coverage generally runs 5:00 p.m. to 8:00 a.m.
- An after-hours triage service screens calls before escalating provider-level issues
- Typical weekday volume is approximately 0 to 1 calls per night
- Physician backup is available for nurse practitioner call
Why Consider PACE?
If you enjoy the complexity and relationships of geriatric medicine but want an alternative to traditional high-volume primary care, this role offers something increasingly difficult to find: Fewer daily visits. Longer appointments. A multidisciplinary team. And the opportunity to know your patients well enough to make a meaningful difference in how they live, not simply how their disease is treated.Qualifications
